What is clinical soft tissue work?
Clinical soft tissue work is the manual evaluation and treatment of problems in the connective tissue. It augments the body’s natural orthopedic healing by taking advantage of the tissue’s inherent malleability.
What goes wrong?
These tissues (muscles, tendons, ligaments, etc…) are often functionally damaged by strain and/or trauma. It can be sudden and violent, as in a car accident. Or it can be repetitive and gradual, as in a tennis elbow.
In either case, the various tissues go through chemical and functional changes that can lead to lasting physical limitations. Typically, the body just deals with it. Either by forcing you, through pain, to modify your behavior. Or by relying on some other as yet undamaged structure in what we clinical nerds call the kinematic chain.
Drugs are reliably effective in relieving pain, thereby making us feel like nothing’s wrong. But outside a few specific clinical circumstances, (like and inflamed disc that’s irritating an adjacent nerve) I encourage people to restrict their drug use to purely recreational purposes… just kidding.
How to make it right?
Fortunately, these soft tissue problems are predictable, palpable, verifiable and reversable.
Predictable:
All bodies are made of the same bones and sinews, subject to the same laws of physics, and heals by means of the same biochemistry. So just knowing what has happened gives the experienced clinician a pretty good idea of what’s going on in the body.
Palpable:
There’s a wonderful German word, “Fingerspitzengefuhl” with an umlaut over the “u”. It literally translates to “perception in the tips of one’s fingers”. But it’s deeper than that. It refers to a finely tuned awareness that borders on the mystical. The great German general Erwin Rommel wrote about how a general needed to be up front and in the danger to achieve fingerspitzengefuhl. Without it, one can’t make good decisions. You’ll recall how one of the Weird Sisters in MacBeth alarmed, “By the pricking in my thumbs, something wicked this way comes!”. That’s Fingerspitzengefuhl. And every manual rehab clinician depends on it as much as his book knowledge.
Without getting too deep into the nerdy specifics, properly functioning muscle has a certain suppleness to it. While strained muscle has taught bands that feel more like beef jerky than anything else. Tendons, ligaments, and joint capsules develop a kind of graininess to their texture.
What should I expect during my first visit?
The first treatment starts with a detailed intake. So expect to answer a series of questions. If there are x-rays or an mri, I would want at least a copy of the written report… preferably the images as well. Then I’d apply various provocative tests to hone in on the compromised tissues. Once I’ve developed a specific hypothesis, I manually treat the compromised tissues to restore function. Improvement upon retesting confirms that we’re on the right track. You’ll also feel the changes in your body. It’s often compared to what the oil can does for the Tin Woodsman in the Wizard of OZ.
